Adult (16 and older)
- 1 Passport Photo (2x2, Color, White Background)
- Proof of Citizenship (Birth Certificate, Passport, or Certificate of Naturalization)
- Government Issued ID (Driver’s License), and photocopy of ID front & back
- Check or Money Order for the Passport Fee (shown below)
Include Expedite Fee and Overnight Fee in check or money order (shown below)
- Cash or Credit Card for the Execution Fee (shown below)
Minor (15 and under)
- 1 Passport Photo (2x2, Color, White Background)
- Proof of Citizenship (Birth Certificate, Passport, or Certificate of Naturalization)
If renewing you need both your Passport and Birth Certificate
- Proof of Parental Relationship (birth certificate, etc.)
- Both Parents need to be present, if one parent cannot be there provide DS-3053
If there is only one legal guardian provide custody documents
- Government Issued ID (Driver’s License, State ID Card, etc.), and photocopy of the ID front & back
- Check or Money Order for the Passport Fee (shown below)
Include Expedite Fee and Overnight Fee in check or money order (shown below)
- Cash of Credit Card for the Execution Fee (shown below)

Passports take approximately 4-6 weeks to be mailed to you by Priority Mail. If expediting, it will be mailed in approximately 2-3 weeks.
PLEASE NOTE: If you are traveling in
14 days or less, you MUST make an appointment immediately with the Miami Passport Agency. Call (877) 487-2778. Be sure to bring proof of travel when going to Miami.
